School-Based Health Centers

The School-Based Health Center is a unique venture with the Utica and Waterville School Districts. With no out-of-pocket expense to you, the School-Based Health Center provides comprehensive primary health care services exclusively to enrolled students in participating schools. Staffing includes a full-time provider, a supervising physician, and a full-time licensed practical nurse. We have established five School-Based Health Centers (SBHC) in the Mohawk Valley. An SBHC provides primary care and preventative health care services in school to those enrolled in the program. 


Students must attend one of the three schools listed below and parents must complete an enrollment packet in order to receive healthcare services. Click here for access to the enrollment packet. 

We offer weekday appointments before, during, and after school throughout the school year. We are closed when school is not in session, including holidays and summer break. 


At our School-Based Health Centers we offer:
 

-Comprehensive physical examinations

-Sports physicals

-Sick child visits

-Nebulizer treatments

-Immunizations

-Mental health screening and referrals

-Oral screening for dental referrals

-Diagnostic treatment for acute and chronic illness

-Health education

-Referral services

-Vision and scoliosis screening

-First aid
