Who We Serve

We provide services to children and adults regardless of insurance coverage or ability to pay.

Our goal is to offer quality primary health care services to you and your family right in your community, eliminating the need to travel longer distances and/or the barrier of payment when accessing this care.

Cost for Services

Upstate Family Health Center, Inc. is a participating provider with the following health plans: Medicaid, Medicare, Fidelis, Excellus, United Health Care, MVP, Empire, Child & Family Plus Plans, and most private insurance companies. We also offer a sliding fee scale based on eligibility. Our staff will assist you in finding the best method of payment to suit your needs. Our goal is to ensure that the ability to pay does not hinder you from accessing the care you need. Our office accepts all forms of payment including cash, check, debit and credit card.
