As our patient, you have the right to:

  • Receive quality healthcare in a safe and respectful environment.
  • Be treated without discrimination.
  • Participate in decisions about your care.
  • Receive information about your diagnosis, treatment options, and medications in a way you can understand.
  • Have your privacy and confidentiality protected.
  • Access your medical records and understand your rights under HIPAA.
  • Receive language assistance or accommodations for disabilities at no cost.
  • Express concerns or file a complaint without fear of retaliation.

As a patient, you are responsible for:

  • Providing accurate and complete health information.
  • Following your agreed-upon treatment plan or discussing concerns with your provider.
  • Treating our staff, providers, and other patients with courtesy and respect.
  • Keeping scheduled appointments or notifying us as soon as possible if you need to cancel or reschedule.
  • Providing current insurance and contact information.
  • Understanding your financial responsibilities and asking questions if you need assistance.

Important Reminder

To help us provide the best care possible, we kindly ask that you let us know in advance if you need to cancel or reschedule your appointment.
Missed appointments without notice are considered "no-shows." Patients who accumulate three (3) no-shows within a 12-month period may be subject to a review of their care plan, which could lead to discharge from the practice. We understand that life happens—please call us ahead of time if you can’t make it. Your communication helps us care for all our patients more effectively.
