Sliding Fee Discount Program
Thank you for choosing Upstate Family Health Center, Inc. for your care. It is our goal to make your experience with us as pleasant as possible.
We offer a “Community Discount" to any patients that qualify by having no insurance, have insurance but high deductibles, have insurance but find it difficult to pay balances
If you are certain that you will not meet the eligibility requirements, or if you have insurance coverage of which we are not aware, or if you wish to set up payment arrangements, please contact our Financial Counselors at(315) 624-9470. Please be advised that the FQHC sliding fee scale will cover dates of services 6 months prior to the date of application and 12 months after the approved date. The determination for eligibility is effective for up to 1 year based on the nature of your income. New assessment and application will be required to be completed EVERY year to continue eligibility or anytime there has been a significant change in family income. Please note that sliding fee scale includes all providers employed by Upstate Family Health Center, Inc.
There are several simple steps required to apply for the program.

- Provide copies of income for all eligible people living in your household (i.e. unemployment stubs, pay checks, etc.)
Services covered under the Community Discount Program include:
- Family Medicine
- Routine preventative exams
- Well child care and immunizations
- Diabetic Care
- Physical exams
- Gynecological care
- Podiatry
What does the community discount mean for you?
As a Federally Qualified Health Center (FQHC), Upstate Family Health Center accepts all Medicare and Medicaid insurance plans, as well as most major insurances. The Sliding Fee Discount is in place to meet the needs of the uninsured or underinsured, providing reduced costs on most services for those who qualify. No one will be denied access to services at Upstate Family Health Center, as services are offered regardless of insurance status or ability to pay.
Community Discount Program
We offer the Community Discount to all income-eligible uninsured or underinsured patients, based on annual household income, for most services. Income guidelines and acceptable verification for the Sliding Fee Discount are listed below
How do I qualify?
To qualify for the Discount, you MUST show proof of gross annual income for all immediate family members living in your household. Gross income is ALL income from ALL sources before taxes. Applicants should provide a copy of any of the accepted income verification materials listed below.
Acceptable Income Verification
- Recent Federal Tax Return
- IRS Form W-2 or 1099
- Two (2) Current Pay Stubs
You will need to apply for this program every year. Your discount will last 12 months. You will need to complete the same application and provide proof of income.
How do I pay for my visit?
Our patients that qualify for our community discount can pay for each visit at the time of service. All forms of payment are accepted in our office, to include cash, check, debit and credit card.
