Upstate Family Health Center, Inc. is a 501(c)(3), not for profit, Article 28, licensed health care center offering family health care services to individuals of all ages at various locations throughout the Mohawk Valley. The experienced and dedicated staff provide the highest level of care, while ensuring that the patient’s needs come first. A patient can receive a continuum of care that focuses on the patient as a whole, providing comprehensive health care for the individual and family.
Upstate Family Health Center, Inc. accepts Medicaid, Managed Medicaid, (Fidelis, Blue Options, United Health Care), Child & Family Plus Plans, Medicare and most private insurance companies. We also offer a sliding fee scale based on eligibility. Our staff will assist you in finding the best method of payment to suit your needs. Our goal is to ensure that ability to pay does not hinder your accessing the care you need.
Upstate Family Health Center, Inc. is deemed by the Bureau of Primary Health Care of the U.S. Department of Health and Human Services (HHS) to be a federal employee for purposes of medical malpractice claims and is qualified for protection under the Federal Tort Claims Act (FTCA).

As a patient, you are responsible for:

- Keeping scheduled appointments or notifying us as soon as possible if you need to cancel or reschedule.
- Providing current insurance and contact information.
- Understanding your financial responsibilities and asking questions if you need assistance.

- If you have insurance, please bring your insurance card or other proof of insurance.
- If you do not have insurance, you may stop by our clinic every Thursday from 1pm to 4pm to meet with an Insurance Navigator

Important Reminder
To help us provide the best care possible, we kindly ask that you let us know in advance if you need to cancel or reschedule your appointment.
Missed appointments without notice are considered "no-shows." Patients who accumulate three (3) no-shows within a 12-month period may be subject to a review of their care plan, which could lead to discharge from the practice. We understand that life happens—please call us ahead of time if you can’t make it. Your communication helps us care for all our patients more effectively.
