Who We Serve
We provide services to children and adults regardless of insurance coverage or ability to pay.
Our goal is to offer quality primary health care services to you and your family right in your community, eliminating the need to travel longer distances and/or the barrier of payment when accessing this care.
📢 Important Reminder for Our Valued Patients
To help us provide the best care possible, we kindly ask that you let us know in advance if you need to cancel or reschedule your appointment.
⏰ Missed appointments without notice are considered "no-shows."
Patients who accumulate three (3) no-shows within a 12-month period may be subject to a review of their care plan, which could lead to discharge from the practice.
We understand that life happens—please call us ahead of time if you can’t make it. Your communication helps us care for all our patients more effectively. 💙

We are located in the large gray building that also houses United Auto Supply and Orkin Pest Solutions. Our front door faces south towards Oswego Street.

To move forward with the new patient process, please complete the following steps:
1. Complete the enclosed New Patient Packet, which includes:
o A request to release your previous primary care and/or specialty care medical records.
o RHIO (Regional Health Information Organization) consent form.
Please note: Submitting the New Patient Packet does not automatically make you an active patient at our clinic.
2. Once we receive and review your medical records, a member of our team will contact you to:
o Schedule your new patient appointment.
o Discuss any additional care plans that may be necessary.

Please return the completed forms by mail or drop them off at our front desk during business hours (Monday–Friday, 8:00
AM – 6:00 PM).
